Family Camp Medical Form can be downloaded here. All campers (adult and child) who attend a Family Camp Weekend in the summer time at Ockanickon are asked to complete this simple health history form with emergency information. Because this camping experience is less than 72 hours a physical exam and doctor's signature are not required. |
